Letter Addressing Registration as an Early Cardiac Care Center

Dear Visitor,

The Chest Pain Emergency Room as an Early Cardiac Care Center serves as a excellent way to provide care for the heart attack patient at a stage most beneficial to him/her. It is with this thought in mind that we congratulate you for establishing a Chest Pain Emergency Room and moving in that direction.

We would like to recognize your efforts by sending you a Certificate of Achievement and place your hospital's name in our National Registry as a charter member so that we may give you proper credit.

Please complete the enclosed application. We will register your hospital, and send the certificate to you for framing to be placed in your Chest Pain Emergency Room.

To register as an Early Cardiac Care Center the minimal requirements are:

1. To have an Early Cardiac Care Outreach Program emphasizing chest discomfort as a meaningful risk factor to check out.

2. To have a "user friendly" place within the Emergency Room to have this accomplished.
